Overview

Processing of a lost Philippine passport involves a working-day waiting period from submission of complete requirements and payment receipt. The Consulate office will interview all applicants for lost-passport replacement. Applicants should bring original supporting documents to avoid delays; these may be verified and returned after completion. Photocopies of the additional required documents are recommended to ensure sufficient copies.

  1. 1Duly accomplished passport application form with complete entries; parents may complete it for minors.
  2. 2Four photographs (1.77" x 1.37") from the last six months showing front view, decent attire, without eyeglasses; digital photos not accepted.
  3. 3Proof of actual residence (driver's license, state ID, employment ID, or similar).
  4. 4Original and authenticated birth certificate from the National Statistics Office (NSO).
  5. 5Supporting documents establishing Filipino citizenship (marriage certificate, school records, professional ID, or similar).
  6. 6Notarized Affidavit of Loss signed before a Consulate Officer detailing the circumstances, passport number, date and place of issue; a police report is required.
  7. 7Photocopy of the lost passport's data page or a previously issued passport (if available).

Payment of Fees

The fee for the replacement of a lost Philippine passport, Rs. 8,352/-, is to be deposited in the Standard Chartered Bank.
